HEALTH FORM REQUIREMENTS 2024 – 2025
PRE-SCHOOL (new to SDJCAcademy for the first time)
- Health Exam/Immunization Form (performed and completed by medical professional) (due BY the first day of Pre-School) with all necessary signatures. Student will not be admitted to school unless we have the form.
INCOMING KINDERGARTEN
- Health Exam/Immunization Form (performed and completed by medical professional) (due BY the first day of school) with all necessary signatures. Student will not be admitted to school unless we have the form.
- Vision – (due BY October 1, 2024)
- Dental (due BY May 15, 2025)
INCOMING GRADE TWO
- Dental Exam (performed and completed by certified DDS) (due BY May 15, 2025)
INCOMING GRADE SIX
- Health Exam/Immunization Form (performed and completed by medical professional) (due BY the first day of school) with all necessary signatures. Student will not be admitted to school unless we have the form
- Dental Exam – (due BY May 15, 2025)
STUDENTS NEW TO SJS FOR 2024-2025 (ANY GRADE)
- Health Exam/Immunization Form – proof of up-to-date immunizations. Students will not be admitted without form.
- Vision Exam (if applicable to grade entering)
- Dental Exam (if applicable to grade entering)
